Care Management Team

In order to optimize care to our clients, Visiting Angels has developed a unique care management system. Our Care Management Team is personally led by Chief Executive Officer and Co-Owner, Nora de Cárdenas. Under her leadership, the Care Management Team performs the following functions and more:

  • Schedules “just the right” aides for our Visiting Angels’ families
  • Responds to the ever-changing care needs of each family
  • Monitors our certified home health care aides
  • Recruits and trains home health care professionals to Visiting Angels’ philosophy of care

RN Supervisors

At Visiting Angels, our NJ-certified home health aides provide clients with personal care assistance; our registered nurses supervise these services. Each client is assigned a registered nurse who assesses home safety, assesses specific care needs, and establishes a personalized care plan to be followed by our home care employees. As services continue, the RN continues to monitor each client by periodically updating the care plan as well as performing non-medical health evaluations. Our nurses are eager to share their wealth of knowledge in order to help direct you to services, assistive products/devices, or other information that may be applicable.

In-Home Assessment

Since every person and situation is different, we must first understand each client’s needs before starting care. This is to make sure our services are relevant and useful. We begin with an in-home assessment.

An In-Home Assessment takes note of each client’s:

  • Cognitive Function: memory, perception, thinking, and reasoning
  • Physical Function: this encompasses the activities of daily living, such as grooming, bathing, dressing, toileting, transferring, and preparing meals
  • Medical Information: medications, medical history, allergies
  • Background: hobbies, interests, special requests

The In-Home Assessment leads to a client’s care plan. This plan acts as a resource guide to our home care employees. As employees interact with a client, following the care plan helps ensure the client’s continued well-being, safety, happiness, and independence.

Our RN supervisors are responsible for non-medical services such as:

  • Conducting in-home initial client assessments and care plans
  • Developing, implementing, and modifying care plans as necessary
  • Conducting follow-up assessments at 60-day intervals
  • Supervision of home health aides in clients’ homes

All of our candidates must pass extensive county, New Jersey state, and federal criminal background checks. We request professional references.
